What is Transition?

Transition is all about you gradually taking more control of your own healthcare as you grow up. Think of it like learning to drive - at first, someone else is in control, but step by step, you learn the skills until you're confidently in the driver's seat. It's not about being left alone or losing support. It's about gaining independence whilst still having people around you who care.

This app is based on the Ready Steady Go programme, developed by Dr Arvind Nagra, Paediatric Nephrologist at Southampton Children's Hospital. The Ready Steady Go programme is widely used across NHS hospitals to support young people transitioning to adult healthcare services. Learn more at readysteadygo.net.

Why is it so important?
Research shows that young people who are prepared for transition have better health outcomes, miss fewer appointments, and feel more confident managing their conditions. Plus, the skills you learn - like communicating clearly, asking questions, and making decisions - are useful for life, not just healthcare. You're not just preparing to see different doctors; you're preparing for adulthood.
Your GP - Always There for You
Throughout your transition, your GP remains a constant support. Unlike your specialist team that will change, your GP stays with you for life. Keep them updated about your appointments, any concerns, and changes to your care. They can help coordinate between your old and new teams, provide advice when you're unsure, and smooth the way if any issues arise. Think of them as your healthcare anchor - always there, always on your side.
